Through our care services, we empower our clients and their families through participation, decision-making, and self-ownership of their care.
Plan Management
NDIS plan management is one of three different ways you can select to manage your NDIS plan. The other two options require you to either see only NDIS-registered service providers, or fulfil all the strict NDIS criteria for self-management.
Essentially, if you ask for plan management funding, you can see any service provider of your choice while your plan manager takes care of all the hard stuff.
Support Coodination
We have experienced and committed Support Coordinators whom are there to assist you with your NDIS Goals.
Our Support Coordinators are committed to helping NDIS participants to keep track of their plan utilisation while they reach their goals. An NDIS provider is only as good as its people. Our partners are committed to ongoing training and development of staff. We connect you with NDIS Providers that take the role of empowering NDIS participants very seriously. We recommend, we’ll help you understand what can be achieved using your NDIS plan.
Accomodation
We at HOH offer 24/7 accommodation that does not have a hospital feel but promotes independent living suitable to your needs.
As part of our services, we empower you to make informed choices that will enable you to have social connections and become part of the community.
In shared living, you will experience a family-like feel that comes with safety and security.

Planning Services
At Hands of Hope Care Service we provide planning services to assist our patients and our customers. We provide tailored support solutions to people of all ages, ranging from one hour of support per week to 24 hours of support per day. These support services are as unique as our clients and take their needs and goals into account. Tailored support services aim to bring a bespoke approach to support—there are no one size fits all solutions.
A Tailored Support Solution Includes:
- Consultation Meetings to determine adequate levels of support.
- Assigning of a Client Services Manager / Client Liaison Officer
- Determination of correct support profiles, support plans and risk assessments
- Creation of a “Training Matrix” of required workers. Introducing a team of skilled staff members to deliver the desired supports
Accomodation Services
Supported independent living.
We at HOH offer 24/7 accommodation that does not have a hospital feel but promotes independent living suitable to your needs. As part of our services, we empower you to make informed choices that will enable you to have social connections and become part of the community. In shared living, you will experience a family-like feel that comes with safety and security.
Short-Term Accommodation (Respite).
We offer support so that you can take a break if you are a carer for a person with a disability. Both children and adults with disabilities can use this service. This is a fantastic opportunity for persons with disabilities to meet new people and experience new things, in addition to giving their carers time to rest and concentrate on other obligations.
In-Home support
Our home support personnel offers adaptable support to your family member in your house. Depending on what you need, it can be a few hours each day, evening, or weekend. Our support staff members assist with everyday living activities like household help, shopping, and personal care whether you are living alone or with your family.
Support and Coordination
By assisting you in connecting to services, we can help you put your strategy into action. The task of navigating the process can be difficult. We can simplify this procedure and make sure you receive the finest assistance available. Together, we will work diligently to achieve your objectives while assuring a cooperative effort.
Life/Personal Skills
We assert, “We promote learning new skills and increasing independence.” This assistance can take many different forms, such as matching you with a mentor to help you network or working closely with you to teach you how to manage your finances, take the bus, and prepare meals.
Additionally, we have expertise assisting people as they move on to the next stage of their lives, such as when they first leave home or finish school and begin working. To make these life transitions successful, we collaborate closely with you.
Are you prepare to relocate from your family’s residence or your shared housing to a place of your own? You can acquire the ability to achieve it with the aid of our programs for independent living skills. To ensure your preparedness, we will create our program in collaboration with you.
